Requirements:
- Must be a psychiatrist or licensed nurse with prescriptive authority (with at least 3 years of behavioral health experience)
- Professional malpractice liability insurance policy of $1M:$3M
- Submission of a fully completed, signed CAQH application
- Individual NPI number.
