Summary of Essential Duties:
- Serve as a clinical co-pilot—charting the best trajectory of each patient’s care by accommodating various needs, including medical, psycho-social, financial and practical.
- Coordinate the care and service of the patient population from admission through discharge.
- Assess and plan, implement, monitor, and evaluate options and services to meet an individuals health needs to facilitate a safe and timely discharge to the next appropriate level of care.

Education:
- Graduate of an accredited nursing program required
- BSN or proof of enrollment in a BSN program within 6 months of hire and completion of program with diploma conferred within 2 years required
License/Certification:
- Current, unrestricted California RN license required
- American Heart Association or American Red Cross BLS certification required
- Case Management Certification required within 2 years of hire
Experience:
- A minimum of 2 years of acute care nursing experience required
- At least 3 years of Case Management experience preferred
- Knowledge of medical literature, research methodology, health care delivery systems and financial reimbursement required
