GENERAL SUMMARY: PROHealth is looking for both Full Time and Per Diem (As Needed) LVN’s. LVN’s are compensated at a Per Visit Rate of $40-45 per visit conducted based upon level of experience. The qualified Licensed Vocational Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with PROHealth policy. Learn more about our Company on our website: PRINCIPAL DUTIES AND RESPONSIBILITIES: Understands and adheres to established policies and procedures. Implements the nursing care plan for each patient. Provides nursing services, treatments and diagnostic and preventative procedures as assigned. Initiates preventative and rehabilitative nursing procedures as appropriate. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patients physical or emotional condition. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. Maintains accurate and complete records of observations, treatments, and care of patients. Attends patient care conferences and in-services as scheduled. Responsible for submitting and changes in schedule to Clinical Supervisor on a daily basis. Prepares clinical and progress notes. Prepares equipment and materials for treatments. Ensures Clinical documents are submitted timely and accurately. Assists the patient in learning appropriate self-care techniques. EDUCATION, EXPERIENCE and LICENSES: Graduate of an approved school of practical (vocational) nursing and currently licensed in the State of California. Minimum of one (1) year as an LVN in the Home Health Care Setting. Must have current BLS Certification. Drivers License and active Auto Insurance required. SKILLS AND ABILITIES: Knowledge of medical terminology Excellent Communication skills; ability to be flexible and work well with others. Ability to maintain confidential information. Ability to prioritize and multitask.
