Job Description:
Home Health RN is responsible for traveling to a patients’ home to administer their services and helping patients maintain their independence.
ESSENTIAL DUTIES
- Traveling to patients’ homes to evaluate and assess patients
medical condition as ordered and report any change - Provides, coordinates, and directs home nursing care
according to physician orders and agency policies - Establish, implement, and revise plan of care and reevaluate
patient needs - Travel on average 175-250 miles
- Average 25-35 visits per week
EDUCATION AND/OR EXPERIENCE
- Active/unrestricted RN License
- 2 years of recent home health experience
