We are looking to hire Fee for Service Occupational Therapists for Brooklyn, Queens, Manhattan, Nassau and Suffolk locations.

ACCOUNTABILITY OBJECTIVE
• This position is accountable for the provision of occupational therapy service compliance wit in accordance with Company policies and procedures, applicable healthcare standards and government regulations.

• The occupational therapist strives to proactively meet the needs of, and assist the patient/client in the achievement of individualized, defined and measurable outcomes.

Responsibilities and Duties 1.

Provides occupational therapy services.

a.

Assesses, treats and instructs patient/clients in the development, improvement, and restoration of the performance of necessary functions, compensation for dysfunctions, and/or tempering of disability.

b.

Administers and interprets diagnostic and prognostic tests of function and applied therapeutic procedures.

c.

Evaluates the patient, confers with the physicians, and participates in the development, maintenance, and periodic re-evaluation of the Physicians Plan of Treatment and the Plan of Care.

d.

Utilizes professional skills and judgment in treating patients to increase strength, coordination, range-of-motion, and function of effected extremities and to retrain damaged nerve pathways or to teach compensation utilizing remaining innovations.

e.

Constructs splints to prevent deformities and additional devices to increase independence for the physically impaired.

2.

Provides observations, assessments and training.

a.

Observes, records and reports changes in the patient’s health status to supervisor and physician.

b.

Instructs, counsels and supervises families and other caregivers in the skills which promote maximum attainable independence for the patient.

c.

Develops a maintenance program and provides direction in environmental adaptation and specific homemaking and ADL skills.

d.

Obtains appropriate physician’s orders for changes in therapy/frequency and direction or communicates need to appropriate Clinical Care Manager.

e.

Participates in case conferences Quality Assurance and Quality Improvement Activities and acts as resource to other team members in the identification of patient needs.

3.

Provides supervision as required.

Supervises, instructs and evaluates Home Health Aide performance in implementing occupational therapy services.

4.

Maintains and submits documentation as required by the Company.

Prepares and submits timely written reports of evaluations, visits, summaries, care plans, case conferences as required by the Company.

Submission of clinical documentation, visit slips and daily time summaries/time slips is required for satisfactory completion of an assignment as defined by Company policy.

Failure to provide required documentation on a timely basis, as currently defined by policy, will result in disciplinary action, and in the case of daily time summaries/time slips, the Company will be unable to generate a paycheck due to lack of documentation on time worked.

5.

Participates in agency’s Quality Assessment and Performance Improvement Program (QAPI) and assures participation of all appropriate staff.

Provides information that enables the collection and root cause analysis of data to identify opportunities for improvement.

6.

Maintains a professional appearance, is reliable and punctual, cooperative, and conducts self-according to accepted standards of professional practice in the course of assigned duties.

7.

Provides support and demonstrates understanding of the patient/client’s emotional and spiritual needs.

8.

Understands the emergency/disaster preparedness plan and procedures in place to meet their patient/client’s individualized needs.

9.

Other duties as needed.

Qualifications Education and Experience To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

The requirements listed below are representative of the knowledge, skills, and/or ability required: Academic The candidate for this position will be an Occupational Therapist who has graduated from an occupational therapy curriculum with a Baccalaureate Degree or Certificate in Occupational Therapy approved by New York State Education Department.

The candidate must be an Occupational Therapist who is licensed/certified by and currently registered with the New York State Education Department or who has been issued a valid permit by that department.

Requires acute or homecare experience within the last two years.
