– COMPETITIVE WAGES EVENING SHIFTS AVAILABLE Job Summary: Responsible for direction of resident care in the skilled nursing setting.
Manages licensed and non-licensed clinical staff.
Consults with staff, physicians, management team on nursing problems and interpretation of facility policies to ensure resident needs are met.
Maintains performance improvement activities for Nursing Services and participates in QI activities.
DUTIES AND RESPONSIBILITIES: Provides the oversight of nursing staff and residents on assigned unit, ensures adequate staffing and quality of care is provided for all assigned residents.
Provides support when needed.
Demonstrates knowledge of nursing staff job descriptions, ensures nursing staff are performing assigned job duties.
Demonstrates problem solving skills, is able to identify and solve problems, exhibits being part of solutions in facility while maintaining a positive attitude.
Demonstrates proper documentation skills, cosistantly documents per policy in the resident record, documents the oversight of the wholistic care of the resident.
Represents Nursing Services on various committees as needed or requested i.e., Infection Prevention and Control, and Performance Improvement Plans (PIPs) for QAPI.
Makes rounds on nursing unit multiple times daily, ensures the unit is organized, tidy, in compliance with regulations and has appropriate supplies, equipment, materials needed for the safe care of the residents.
Participates in the performance improvement functions and quality of care by collecting and verifying data, completing audits as directed by the RN/DON.
Consults with other departments, as appropriate, to provide an interdisciplinary approach to the resident’s needs.
Attends required IDT meetings.
Treats residents and their families with respect and dignity.
Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of residents and their families.
Ensures licensed nursing staff can perform skills needed to provide safe skilled services.
Assists in nursing competencies and conducts routine staff evaluations.
Provides support and encouragement to staff, follows and demonstrates the brand integrity values.
Demonstrates knowledge of the CMI quality measures, ensures residents are being provided quality care reflected in assigned resident’s monthly quality indicators.
Ability to identify and respond to residents change in conditions as they occur, consults with medical and other parts of IDT regarding resident changes and implements care plan interventions associated with resident changes and risk factors, includes resident and family in plan of care.
Completes medication passes as needed or directed.
Follow the medication rights and reduces the potential for medication errors, addresses med errors with staff when occur per policy.
Ensures units medication storage, narcotic and medication handling is all in compliance with DOH regulations.
Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.
Supports and maintains a culture of safety and quality.
Supervises, supports and developes licensed nurses in their daily work.
Directs nursing staff to continuing education and inservice programs as needed or requested.
Coordinates and directs resident care by monitoring and communicating updates in care plans, with staff resident and family; ensures care plan compliance occurs on assigned unit.
Follows direction of RN care planning and assessment.
Assists in accurate data collection for completion and updating the Minimum Data Sets and care plans as needed or when directed by RN.
Conducts monthly unit staff meetings with all shifts and daily huddles / report Exhibits genuine interest in and appreciation of adults in the later years of life.
Is an advocate for the residents of the facility.
Is a motivated, independent individual who can organize workload so that all functions are completed appropriately.
Professional Requirements: Adheres to dress code, wears name badge, appearance is professional, neat and clean.
Completes annual education requirements.
Maintains regulatory requirements and active NYS certification.
Maintains resident confidentiality at all times.
Reports to work on time and as scheduled, completes work within designated time.
Wears identification while on duty, uses computerized punch time system correctly.
Completes education/in-services in a timely fashion.
Monthly and annually and as requested.
Attends annual review and department inservices, as scheduled.
Attends staff meetings such as shift huddles/reports.
Represents the organization in a positive and professional manner.
Actively participates in performance improvement and continuous quality improvement (CQI) and QAPI activities.
Complies with all organizational policies regarding ethical business practices.
Communicates the mission, ethics and goals of the facility.

Current Licensed Nurse in State of New York.
