– Buckeye, AZPosition TypeFull TimeJob CategoryHealth CarePOSITION SUMMARYThis position functions to assist all patients and their families through the healthcare system by acting as a health educator and patient advocate.

They will also work with patients to develop preventive and self-management goals to improve their health.

Additionally, this nurse position will provide triage services to patients calling for urgent care needs to provide medical advice and identify if outpatient services are appropriate or if they should be escalated to a higher level of care.

This position is essential to Adelantes team-based primary care medical home model of care that supports coordinated, patient-centered, comprehensive health care to all.

Reviews incoming patient messages via the electronic medical record system and the patient portal.

Then assists in responding to nursing related message and delegation of other messages to supporting team membersAssesses patients learning needs and delivers patient-specific education planCollaborates with patients to set self management goals to help meet their health goalsParticipates in team meetings, designated Specialty Team meetings, and quality improvement initiatives.May facilitate group health education sessions and assist in curriculum development.

Coordinates continuity of patient care with external healthcare organizations and facilities such as referrals to outside specialties and ongoing two-way communication between providers and care teams.

Coordinates outreach efforts to high-risk patients and hospital discharge and admissions.

Will collaborate with health plans, hospitals, and skilled nursing facilities for transitional needs.

Conducts comprehensive, preventative screening for patients and/or assists all support staff in daily patient interactions as needed.

Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans.

Facilitates patient care based upon approved standing orders and protocols.Works collaboratively with the clinical team on data collection, health outcomes reporting, clinical audits, and programmatic evaluation.

Helps evaluate clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
