Hours: Mon-Thurs 1pm-10pm, flex Fri 11am-7pmJOB SUMMARY:Under the supervision of the Assistant Director of Supportive Housing Programs, Family Advocates are responsible for providing coordinated Case Management and Supportive Services for pregnant and parenting non-minor dependents.
The Family Advocate will deliver services on-site to program participants housed in our Transitional Housing Placement Program for Non
– Minor Dependents (THPP-NMD).
The Family Advocate will also help link program participants to services that include but are not limited to life skills workshops, money management, educational attainment, access to health services, counseling, job training, and placement opportunities.ESSENTIAL JOB DUTIES:* Develop a self-sufficiency/case plan with each program participant that describes objectives, action steps, timeframes, and supportive services necessary to achieve the objectives.* Assist program participants with the implementation of the self-sufficiency/case plan and discharge plan by meeting formally once a week with each program participant in their home and informally as often as necessary.* Continuously assess residents to identify changing needs and modify their self-sufficiency/case plan and staff interventions as necessary.* Appropriately document the program participant’s activities and program participation in the weekly home visits forms.* Complete Quarterly Reports and related documents and collaborate with County Social Workers in assessing program participants’ needs and in the development of their Transitional Independent Living Plan (TILP)* Research mainstream/supportive services and facilitate each program participant’s engagement in appropriate services according to their self-sufficiency plan.* Provide crisis intervention to program participants and develop safety plans to assist program participants with making safe choices.* Support clients as they navigate the housing stability process including, support with obtaining needed documents for establishing eligibility, housing search and landlord engagement, referrals to community resources that match client goals, increase in income through employment and benefits.
This includes utilizing the Clarity system to engage with other programs for additional housing opportunities.* Utilize a trauma-informed/harm reduction/housing first and solution-oriented model when working with clients.* Respond to urgent requests for assistance from clients or the property manager as needed.* Complete all forms/reports required for Supportive Housing Programs and submit by the established deadlines.* Routinely conduct apartment checks, common area checks, and complete work orders to maintain apartment and facility safety.* Provide crisis intervention to residents and develop safety plans to assist residents with making safe choices.* Conduct Program Informational/Orientations for program applicants as needed.* Orient new intakes to the program, including completing documentation.QUALIFICATIONS:* Bachelor’s Degree in Behavioral Science and a minimum of two years of full-time experience working with transition-aged youth, or a Master’s Degree in Behavioral Science or Human Service based on current managing contract guidelines.* Must possess and maintain a valid California Driver’s License and a driving record acceptable to the agency’s insurance carrier.* Physical ability to perform Pro-ACT, administer first aid/CPR, and operate an Agency motor vehicle.* St.
