RN Case Manager
We are looking for a local full time/permanent/direct hire RN Case Manager.
This position comes with a $10k sign on bonus .
This position will be based out of a local Nursing Division with local travel to Out Patient/Ambulatory Clinics/ Hospitals/Doctors Offices to provide Case Management duties for this specialized patient population.
This is a full time/perm position and is open with a reputable, financial sound, national healthcare system right here in LA.
Case Management experience in Managed Care, a Health Plan, Hospice, or Home Health environment highly desired. This CM position has a focus on Infectious Disease / Disease Management/ Psych and Social deterrent / Psychosocial needs of the patient.
Monday-Friday 8:00-5:00(some flexibility on this), with no weekends or on call. They provide great work hours, schedules, extensive benefit package, and growth potential. Case loads are kept manageable for quality care.
