Mission Community Hospital is a 145 bed acute care hospital with a 50 year legacy of serving the San Fernando Valley. We currently seek a Full Time Clinical Pharmacist.
The Clinical Pharmacist is responsible for the provision of clinical pharmacy services including: anticoagulation, pharmacokinetics, antibiotic stewardship program, drug dosing consultation, nutritional support monitoring, drug information, formulary reviews, in-services on pharmacology/disease state pharmacotherapy, pharmacokinetics to medical, nursing, and pharmacy staff. This includes the selection of medications to disease state, monitoring of patient’s therapeutic outcomes, follow-up on medication errors and evaluation of adverse drug reactions. Provides prescribed medications, other pharmaceuticals and compounded medications according to professional standards and practices, hospital policies and procedures, regulatory agencies, and Federal and State laws. Performs medication order verification, supervises preparation of all sterile IV medications, narcotic dispensing and accounting, dispensing medications. In addition, may perform drug use evaluation, quality assurance, maintain the formulary, and review/report adverse drug events.
KEY RESPONSIBILITES
Clinical – 35%
- Evaluates patient’s pharmacotherapy for appropriateness of drug selection, dosage, dosage
form, route, and duration, taking into account adverse drug events or medication errors.
- Performs pharmacokinetic dosing and consultations, providing dosage adjustments based on
medical history, diagnosis, weight, age and laboratory test results.
- Performs total parenteral nutrition dosing and consultations, providing dosage adjustments
based on patient’s medical history, diagnosis, weight, age and laboratory test results.
- Actively participates in multidisciplinary ICU rounds to provide clinical support regarding all
medication therapy management.
- Contacts the prescriber, as appropriate, to clarify orders and/or make recommendations
concerning alternate therapies.
- Performs drug utilization evaluation (ie. DUE, MUE) as needed to promote current evidence-
based pharmaceutical care.
- Responsible for formulary management. Suggests cost-effective therapeutic equivalents or
conversion of intravenous therapy to oral therapy whenever appropriate.
- Initiates and documents clinical interventions and follows up on all interventions left from
previous shifts.
Education – 20%
- Identifies educational needs of the medical, pharmacy or nursing staff in order to improve
patient care activities related to medication usage.
- In collaboration with the Director of Pharmacy performs in-services and training for
pharmacy, nursing and medical staff.
- Participates in multidisciplinary patient care rounds for the purpose of providing pharmacy
input on patient care planning.
- Maintains and develops clinical skills by attending conferences, seminars and reviewing current literature and periodicals.
Distribution – 35%
- Responsible for compliance with all applicable policies and procedures, and standards of the
hospital.
- Maintains patient confidentiality as per HIPPA regulations.
- Reviews, clarifies, and verifies all physician orders for appropriate dose, efficacy, safety,
drug interactions, and incompatibilities.
- Supervises technician functions. Is knowledgeable of technician responsibilities and their time commitments. Checks accuracy of technician distribution functions.
- Performs and supervises IV sterile compounding. Maintains all logs and required documentations.
Inventory Management/Medication Storage – 10%
- Controls drug purchasing costs by utilizing purchasing agreements established through the hospital.
- Controls drug costs through formulary management, therapeutic streamlining programs, clinical interventions, and controlling outdates.
- Monitors the procurement and receipt of medications and supplies and the maintenance of par levels.
- Ensures the appropriate medication storage requirements are met.
- Ensure continuous monitoring of medication storage within the hospital.
QUALIFICATIONS:
Required
- Current California Registered Pharmacist License
- A Doctor of Pharmacy degree from an accredited school of pharmacy
- Completion of ASHP-accredited PGY1 residency training in an acute care setting
- Minimum of 1 year of acute care hospital experience
Preferred
- Board Certified Pharmacotherapy Specialist (BCPS)
