The Medical Case Management Supervisor blends the mission of AHF with Case Management the oversight at a first-line management level for the Los Angeles County Ryan White EMA.
Utilizing a strong understanding of Ryan White Medical Case Management Services, a thorough understanding of the Ryan White system and the Los Angeles County Standards of Care, you will work with our providers and healthcare center team to ensure that your Case Management services are offered and executed in the prescribed manner to the Ryan White eligible population in Los Angeles.
You will work with both MCC, HCC staff and providers so that our patients receive the support they require and access to available resources.
You will provide guidance and operational assistance to the team and monitor services entered into the Case Management Information system.
You will use the monitoring tool provided the Ryan White contract to be sure those measures that will be audited are being completed by your team.
By conducting 1: 1 meetings with your team, you will understand the barriers and challenges and work to resolve.
In this way you will support the team’s ability to help their patients achieve their healthcare goals.
You will participate in external programs, consortium’s, cohorts at the local level and beyond..
You will also collaborate with the Associate Director for Case Management Operations for growth in Los Angeles County.
You will bring the essence of the Ryan White Care Act
– the safety net for the uninsured HIV/ AIDS patients
– to the eligible population and to your AHF team including providers, HCC Ops team and Managed Care for ongoing discussion and understanding.
You will review the deliverables so they remain on target.
Beyond the scope of the contract, your team will contribute to the measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.
Your oversight of the Los Angeles Case Management Program and your participation in external support groups, consortiums etc adds to the AHF mission so that client/ patient experience in Case Management is optimum..
Registered Nurse or Bachelor’s degree preferred or.
Master’s degree in nursing or social sciences with 5 years’ work experience in HIV/AIDS preferred..
In addition, the Manager of Medical Case Management will have an extensive:.
Knowledge of community resources.
Knowledge of target population.
Knowledge of HIV disease and treatment.
Competence in Cultural and linguistic Diversity..
